Simply Skin Studios LLC, doing business as Elemntal Wellness ("Elemntal Wellness," "we," "us" or "our"), operates this website and our wellness studio at 360 W Broadway, Suite 102, Salt Lake City, UT 84101. This policy explains how we handle personal information when you visit elemntalwellness.com, contact us, book or receive services, purchase a membership or package, sign a waiver, or receive our communications.

2. Information we collect

• Contact and account details: information you provide, such as your name, email address, telephone number and client account information.
• Booking and purchase details: selected services, appointment times, provider preferences, memberships, packages, transaction records and payment-related information processed through our booking and payment providers.
• Signed waivers: your signature and the identifying information and acknowledgments recorded on a waiver covering services we provide.
• Communications and preferences: messages you send us, our responses, communication preferences and records of marketing consent or opt-out requests.
• Website usage information: information such as device and browser details, IP address, pages viewed, referring pages, interaction events and cookie or similar identifiers, collected through hosting, analytics and advertising technologies.

We do not collect health histories or health information as part of our routine client intake. We do collect signed service waivers. Please do not send medical records or other sensitive health information through general email, text messages or website inquiries.

We use information to respond to questions; arrange, provide and manage services; maintain client accounts and signed waivers; process purchases and memberships; send appointment confirmations, reminders and service updates; and send promotional messages in accordance with your communication choices and applicable requirements.

We also use information to operate and improve the website, understand visitor activity, measure advertising performance, protect our business and clients, prevent misuse, maintain appropriate business records and meet legal obligations.

4. Service providers and disclosures

We do not sell customer contact lists or give customer contact information to other businesses or affiliates for their own direct marketing or promotional use. We do use outside providers to operate our services, website and communications:

• Boulevard: online booking, client accounts, payments, client management and appointment or marketing communications.
• GoHighLevel: customer relationship management, follow-up and messaging, including marketing communications.
• Hosting, payment and technical providers: services needed to host the website, process transactions, deliver communications and support operations.
• Google Analytics and Meta Pixel: website analytics, advertising measurement and related uses described below.

These providers may receive or process information needed for their functions. Their handling of information is also governed by applicable agreements and their own privacy notices. We may disclose information when required by law, in response to valid legal process, or as reasonably necessary to protect legal rights, safety and security.

The use of analytics and advertising technologies involves disclosures of device identifiers and website activity to their providers. Those disclosures are distinct from selling customer contact lists, and are described in the following section.

5. Cookies, analytics and advertising

Our website uses Google Analytics and Meta Pixel. These tools can use cookies, pixels and similar technologies to collect information about visits and interactions, including device or browser identifiers, IP addresses, pages viewed and events such as clicks. Google Analytics helps us understand website use. Meta Pixel helps measure advertising results and may support relevant advertising on Meta services.

6. Text messages and email

We use Boulevard and GoHighLevel to send appointment confirmations, reminders, service-related communications, promotional text messages and marketing emails. Promotional messages are sent based on your consent and communication preferences, as applicable. Consent to receive marketing texts is not a condition of purchasing services.

Message frequency may vary. Message and data rates may apply. Reply STOP or UNSUBSCRIBE to the number that sent a message to opt out of that text messaging program. Reply HELP for assistance, or contact us at (801) 573-6923 or info@elemntalwellness.com.

Because messages may be sent through more than one platform or number, an opt-out sent to one program may not update a separate program. Contact the studio if you want help updating all of your preferences. You can unsubscribe from marketing emails using the unsubscribe link in an email. Essential service or transaction-related emails may continue where appropriate.

7. Services for minors

We may provide services to individuals under 18 with parental or guardian consent. A parent or guardian should arrange the minor's appointment and provide the required consent and waiver. This website is intended for adults arranging services, rather than for children to submit information independently.

If you believe a child has provided personal information without appropriate parental or guardian consent, contact us so we can review and address the situation.

8. Retention and security

We retain personal information for as long as reasonably necessary for the purposes described here, including managing client relationships, documenting consent and signed waivers, maintaining transaction records, handling disputes and meeting applicable legal obligations. We use reasonable administrative, technical and organizational safeguards appropriate to the information we handle. No online transmission or storage system can be guaranteed completely secure. Please protect your client login credentials and contact us if you suspect unauthorized access.

Depending on where you live and which laws apply, you may have additional rights, including rights relating to targeted advertising or certain disclosures of personal information.
